What you’ll need

The form takes about five minutes. It helps to have these details ready:

  • 1Your childFull name, date of birth and city of birth.
  • 2Both parentsNames, address, religion, email and phone number.
  • 3The godparentsFull names of the godfather and godmother, and their religion.

Your information is kept confidential and used only by the parish office to prepare for your child’s Baptism.
