What you’ll need

The form takes about ten minutes. It helps to have these details ready:

  • 1About each of youName, religion, date and place of birth, occupation, address, email and phone.
  • 2Your parentsFull names (mother’s maiden name) and religion, and whether either of you has been married before.
  • 3Witnesses & dateNames and addresses of your two witnesses, and your wedding date.

Your information is kept confidential and used only by the parish office to prepare for your wedding.
