What you’ll need

This form is for roles that need full screening, such as work with children, seniors or other vulnerable people, or with money or confidential records. It takes about ten minutes. The first part asks about you, and the second asks for two character references.

Your information is kept confidential and used only by the parish office. Once submitted, someone from the parish will contact you to follow up.
